I'm sure I'm not getting something but somehow I'm not able to set a customItem in all nodes In the topnode, and the subgroup 1st level it is working ok but I somehow always get lost doing it in the nodegroups and the nodes below that.

Also as I'm a very basic and occasional user of your - otherwise great - toolbox, is there any way of getting some additional info on using it (eg a more extensive manual, either on paper or in electronic form)

You will always use CustomItem with a node, not a node group. In case you need to fill a node group with a CustomItem, you would need to create a dummy node inside that node group first, then associate your CustomItem to that dummy node.
