When I convert my webpage to a pdf locally then checkboxes appears fine, but on the deployment site the checkboxes show up as black squares when converted to a pdf.

Hi,
Please try to update to the latest build first. We have fixed an issue that may be related to your issue where the problem is related to themes and it would behave differently when themes are enabled but the process is running interactively or running without a console (for example, as a service). I believe that issue was fixed. So try the new build and see if it fixes the issue for you.
Note the new build is release 2012 as oppose to 2011, in order to use the new release, you must go to your license key page to get a new license key (it's free for you) and apply the new license key.
If the new build still does not work for you, we would need your help on figuring out the steps to reproduce the problem. Usually as soon as we can reproduce it, we can get to the bottom of it.

Still have the same issue after upgrading to the newest dll. Seems like it may be something related to css. If I completely remove the css then all input fields are black.

Hi,
We are not aware of CSS can affect checkboxes. Please try to figure out exact steps to reproduce the problem. If you need to send us test file please also let us know and we will give you the email address. We will need to be able duplicate the problem in our environment in order to investigate further.
